Application Forms

Submit your information to our online directory of artists and performers, or let us know how you'd like to participate in the next Festival:

Our application forms are very simple to use. Please make sure to fill in all required information accurately.

If you are an artist or performer, when you fill in the form, you will be able to choose whether you are applying to be in the next Festival or to be listed in our online directory of artists and performers -- or both. Just follow our simple instructions.

If you're applying to be in our online directory of Chicago area Jewish artists and performers, please come back to our site in a couple of days to view your entry. This is a great way to let people know about your work and how to contact you, so pay careful attention to describing what you do.

All forms can either be submitted online or printed and mailed. If you have any questions, please call us at 847-933-3000 or send us an email.

If you are not submitting your Application Form online, send it to:

The Greater Chicago Jewish Folk Arts Festival
P.O. Box 5215
Skokie, IL 60076

  • Volunteer Applications:   If you would like to volunteer for the next Festival AND have a great time, please send us an email or contact us at 847-933-3000. One of our staff will get back to you promptly!
  • Co-sponsoring Organization Application Form (click): A major reason for the tremendous success of the Festival is the support of co-sponsoring organizations like yours. Through the efforts of organizations we are able to reach thousands of people with our publicity and truly bring together the Jewish community. To qualify as a Co-Sponsoring Organization, your group must satisfy the purposes and objectives of the Festival as set forth in its Rules for Participation.

As we prepare for this year's Festival we welcome your participation as a co-sponsoring organization. As a co-sponsoring organization you are expected to:

    • Mail flyers to your entire membership (we will deliver them to your office); if this is not possible please contact us. However, it is extremely important to us that we get as many flyers to your members as possible.
    • Include the information about the Festival in your monthly calendar or newsletter.

In exchange for this help with the Festival, we will:

    • List your organization in the Festival program.
    • Offer your organization space at the Festival to publicize your work and interest new members. (There is a table fee).

Be part of the event which expresses the heart and soul of Jewish Chicago. Complete the Co-sponsoring Organization Application Form on line or print it out and mail the printed copy to P.O. Box 5215, Skokie, IL 60076 as soon as possible. In either case you will need to mail a check for the space and table fee as soon as possible. Your participation will help make the Festival a great community event. If you have any questions, please call 847-933-3000 or email us with your question.

  • Artist Application Form (click)The Festival is a showcase for Jewish artists and/or Jewish art -- and our web site houses a year-round directory to Chicago area Jewish artists, many of whom have been Festival participants.
Entries to the Festival must be original works and can be produced in any media. The Festival has over 100 artists from around the U.S. and other parts of the world. If you are an artist's representative or distributor you must indicate so on your application. The content of the art work must be consistent with the purpose and objectives of the Festival as set forth in its Rules for Participation. The Festival is a juried art fair and the jury chooses artists based on concept, originality, Jewish content and quality of workmanship. Artists must submit images to the jury consistent with the work to be exhibited on the day of the Festival and cannot include media or works by other artists not previously juried.

Artists may submit applications either online or by mail.

Six foot tables are available at a cost of $20.00 each. Artists are responsible for display boards, chairs and any preparations needed for inclement weather. There are NO refunds due to weather related conditions. Artists will be advised on the morning of the Festival if the indoor location will be used.

The art committee looks forward to hearing from you. If you have any questions, please call 847-933-3000 or email us at ChiJewishFest@aol.com with your question.

Please complete the online application, print out and mail in with a sample tape (video or audio) of your work. Send the application and tape to: Greater Chicago Jewish Folk Arts Festival, P.O. Box 5215, Skokie, IL 60076.

If you have any questions regarding auditions, please call 847-933-3000, or email us with your question.
